Although the doctor at the hospital said that I was well enough to go home, I would still need assistance in my daily activities.
When I am at home, my family will be trained and helped by the Ren Ci - Code 4 Home Care team comprising Doctors, Nurses and a Physiotherapist. They will help me and my caregivers learn to be more independent in caring for myself and improving my quality of life.

Eligibility:
Seniors aged 55 years or older who:
are homebound due to physical and/or mental disabilities
have medical problems requiring assessment and follow-up
live in the eastern and central zones of Singapore
Charges:
Services fees, charged per visit, will depend on the financial circumstances of the patient.
